United Care Medical Transportation ("United Care," "we," "us") respects your privacy. This Privacy Policy explains how we collect, use, disclose, and safeguard your information when you visit our website or use our services.
We may collect personal information including your name, contact details, transport addresses, insurance details, and protected health information necessary to provide non-emergency medical transportation services.
We share information only with healthcare providers, insurance carriers, and authorized parties as necessary to provide and bill for services. We do not sell personal or health information.
We use industry-standard encryption, secure dispatch systems, and HIPAA-compliant infrastructure to protect your information. Access is restricted to trained, authorized personnel.
You may request access to, correction of, or deletion of your personal information at any time by contacting privacy@unitedcaremt.com.

By booking transportation with United Care, you agree to these Terms of Service.
Bookings are confirmed in writing. Cancellations made within 2 hours of pickup are subject to a $25 fee. Same-day reschedules are accommodated when possible.
We commit to safe, professional, on-time service consistent with industry best practices and applicable state regulations. Service is not emergency medical transportation; for emergencies, call 911.
Payment is due at the time of service unless billed to insurance or an established facility partner. Late payments may incur additional fees.
United Care carries $5M liability insurance. Our liability for any claim is limited to the cost of the transport in question, except where prohibited by law.
These terms are governed by the laws of the Commonwealth of Pennsylvania.

United Care is committed to protecting your medical information. We are required by law to maintain the privacy of your Protected Health Information (PHI) and to provide you this Notice of our legal duties and privacy practices.
If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer at privacy@unitedcaremt.com or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
